Each document arrives twice over: a PDF as the main thing to read, and a plain-text version of the same content for copying a section into another tool (such as an AI chat), or editing yourself.

The Source Documents Come Too

The structured documents above are the synthesis. The underlying source-of-truth is included alongside them: the original documents released by your GP practice (typically as pdf files).

GP records from before practices digitised (roughly 2000 to 2010, depending on the practice) usually arrive as scanned handwritten notes: searchable by no one, legible to almost no one. You receive a typed transcription of every such page, sat alongside the original scan so any reading can be checked.

Quality Control

How we work, not just what you get. These are principles we hold ourselves to as we build your pack.

Leading models, deliberately chosen

We use the leading AI language models available, picked for each step rather than spread thinly across the whole pipeline. As stronger ones arrive, we move to them. The field changes month by month, and your record deserves what's good now, not what was good two years ago.

Built in stages, checked against the source

We don't ask AI to produce your pack in one go. It is built in stages: extract, organise, draft, verify, revise. Every fact ends up traced back to the page in your GP record it came from. Doing it all in one shot is faster and cheaper, but the result isn't the same.

Discrepancies surfaced, not smoothed over

When sources in your record disagree, both versions are recorded with a clear resolution rationale. Conflicts are surfaced, not silently reconciled.

If you spot something off, we fix it

Every cited fact points back to a page in your original record, so anything you doubt can be checked in seconds. If something looks wrong, please let us know.

Reasonable endeavours, openly stated

No organisation of a complex medical record will be flawless, and the pack is not a substitute for clinical advice. We aim high and are honest about the limits of what we're able to achieve.
