Gift a Loved One Their Health Record

How It Works

A GP record must be requested with the permission of the person whose record it is. Nothing about anyone’s health is gathered when you buy. The person you give it to redeems it themselves and gives their own authority to gather their record. They stay in control the whole way through.

1. You buy the gift. Payment is taken securely upfront. Within a few minutes you receive an attracted branded certificate by email, with a unique code on it.
2. You pass it on. Forward the email, or print the certificate, and give it to whoever you like. It is not tied to an email address, so if the person you had in mind would rather not use it, it can simply be passed to someone else.
3. They redeem it. When they are ready, they email us the code. From there it is the normal Chronicle Health process: we confirm their own authority for their own record, gather it, and build their pack. There is nothing further to pay.

The Essentials

• One certificate covers one complete personal health record service.
• Valid for 12 months from the date of purchase, and we will honour reasonable redemption sympathetically beyond that.
• Transferable: whoever holds the certificate can redeem it.
• Non-refundable once redeemed or once work has begun, except for your statutory cancellation rights as a consumer. The detail is in the Gift Purchases section of our Terms of Service.
• The price is fixed at the time you buy. A later change to our fee does not affect a certificate already bought.
• Buying for someone who cannot make their own decisions, or arrangements involving a lasting power of attorney, are not covered by a standard gift and need to be arranged with us directly.
