Here is how the process looks from start to finish.

I'll listen to what you want, show what a finished personal health record looks like, discuss what it enables, and answer any questions. There is no commitment at this stage.
If you decide you want to move ahead, there'll be a contract to sign, to provide consent.
1 Data collection

There are two options:
- Option A: You submit the SAR yourself using the template and guide I provide.
- Option B: I submit the SAR on your behalf as your authorised representative (requires a signed Letter of Authority and identity documents).
GP practices typically respond within a week, though legally they have up to 30 days.
Your own data (optional)
If you have data from other sources such as wearable exports, private lab results, or manual tracking apps, you can choose to provide those too and I'll organise them alongside your GP record. I can help guide you through exporting data if needed.

Once I've got the data, in a focused processing session I use tools to:
- Remove personally identifiable information (names, phone numbers, addresses) from the raw records
- Transcribe any handwritten clinical notes (GP records from before digitisation are hand-written)
- Clean, organise, and standardise everything into your structured personal health record

I will securely send you the data gathered from your GP and all processed deliverables. You'll download them to keep and use however you like.

4 Aftercare

You'll have ample opportunity to verify the results, ask any questions. If you raise any issues, they'll be addressed. I will then delete all of your health data from my devices. See the Retention Policy →
Optionally, we can also arrange:
- A follow-up call: A chance to ask questions that have come up during use, or give feedback. Typically 15–30 minutes.
- Future updates: To keep your health record current as things change. Not included in the initial service.
What the service does not include
What the service does not include
- ✕ Medical advice. I organise your health data — I do not interpret it clinically.
- ✕ Paper document scanning. If you have paper records, you will need to scan or digitise them before I can process them.
- ✕ Software, cloud services, or AI models. I do not sell any technology. I process your data and you receive it as files that you own and control.
- ✕ Raw genetic sequence data or other medical 'big data'. Underlying sequence files and similar large datasets are outside scope. That said, most consumer services (e.g. 23andMe) produce summary reports — those are a good fit for your record.
