Your complete health history, organised and in your hands
I retrieve your full GP record and transform it into a structured personal health record you own and control.
Get StartedYour health data is a distributed mess. It doesn't have to be.
Your GP record is the closest thing to a lifelong health record in the UK — decades of consultations, prescriptions, referral letters, test results, and correspondence from hospitals and specialists, all in one place. You're told some of these results when they happen and can access some of it via the NHS App, but there's typically much more hidden away.
Most people have never seen their full GP record. With AI-powered health tools improving rapidly, having your data in a structured, usable form matters more than it used to.
What I do
I retrieve your full GP record through a Subject Access Request and transform it into a structured, searchable, referenced personal health record.
What arrives from the GP practice is typically an unstructured PDF of 100s of pages, including scanned documents and clinical data exports. Older records often include handwritten doctor's notes, potentially going back to the day you were born. I clean and organise everything, including transcription of the handwritten records into usable text. I deliver a comprehensive health record that you then fully own and control.
Why it's worth doing
See what's actually in your record
Most people have never read their GP record as a whole. When everything is organised chronologically, you can trace what happened, when, and in what order — including things you were never told or have long forgotten.
Stop duplicating and guessing
With everything in one place, you can check which tests you've already had and which treatments have been tried, instead of reconstructing from memory.
Use your data with AI
A structured health record can be used with AI tools — something that is not possible with the NHS App, scattered PDFs, or raw handwritten notes. Ask questions about your own history in plain English.
Take your records anywhere
Your health record is a set of files you own. It goes with you if you move, change GP, travel, or need to share relevant history with a new provider.

Pricing
For my next 10 clients (from April 22nd): pay what you think the service was worth, after you've received your health record. That includes paying nothing at all. No upfront payment, no minimum, no obligation.
After that, pricing will move to a fixed rate.
Privacy and control
Your health data is sensitive. I handle it accordingly — pseudonymisation early in the process, encrypted storage, zero-knowledge and zero-retention third-party processors, full deletion after delivery. I will never share your data or discuss your health with anyone other than you.
This is a personal health data organisation service. It is not a medical device, not medical software, and not a substitute for clinical judgement. Outputs are personal records — not clinical summaries or diagnosis.
"I feel empowered to have all my NHS records in one place. It's really helpful to know that whenever I need to reference my health history, everything's already there and organised."
— Clare Relton, early client
